Completely sterilized and ready to use. My wife actually did not use the breast shields that came with the pump, so those are completely untouched and never used (brand new). Comes with all the boxes, manuals, accessories. The pump works really well. Just as well as my wife's Madela pump.

However, my wife preferred using Madela because cleaning it is easier. You can use milk bags when you're out and about, and reusable containers when you're at home.
